<em>Escherichia col<\/em>i STEC en fonction de leur potentiel de virulence est propos\u00e9e par l\u2019Anses sur la base des donn\u00e9es \u00e9pid\u00e9miologiques fran\u00e7aises r\u00e9centes et de la litt\u00e9rature internationale, tenant compte de l\u2019association des souches avec les formes s\u00e9v\u00e8res d\u2019infection, principalement le syndrome h\u00e9molytique et ur\u00e9mique (SHU) et la diarrh\u00e9e sanglante.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Quatre groupes sont ainsi d\u00e9finis :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>groupe I : souches poss\u00e9dant les variants stx2a et\/ou stx2d et le g\u00e8ne ou marqueur d\u2019adh\u00e9sion eae (ou aaic\/aggR), pr\u00e9sentant le plus fort potentiel de causer des SHU ;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>groupe II : souches eae-n\u00e9gatives poss\u00e9dant les variants stx2a et\/ou stx2d, plus fr\u00e9quemment impliqu\u00e9es dans les cas de SHU chez les adultes ;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>groupes III et IV : souches poss\u00e9dant les autres sous-types stx et positives (groupe III) ou n\u00e9gatives (groupe IV) au g\u00e8ne eae (ou aaic\/aggR), moins souvent associ\u00e9es aux cas de SHU.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Le s\u00e9rogroupe n\u2019intervient pas dans la virulence et n\u2019a pas \u00e9t\u00e9 retenu comme un crit\u00e8re pour la nouvelle classification des souches.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cette nouvelle d\u00e9finition pourrait remettre en cause, la d\u00e9finition de l\u2019avis de l\u2019Anses du 18 mai 2017 et plus particuli\u00e8rement l\u2019approche actuelle avec 5 s\u00e9rogroupes class\u00e9s comme hautement pathog\u00e8nes : O157:H7, O26:H11, O103:H2, O11:H8,O145:H28 ainsi que les m\u00e9thodes de d\u00e9tection, majoritairement bas\u00e9es sur la d\u00e9tection de ces s\u00e9rogroupes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cependant, dans l\u2019attente d\u2019un texte r\u00e9glementaire ent\u00e9rinant ou modifiant l\u2019avis de <a href=\"http:\/\/chrome-extension:\/\/efaidnbmnnnibpcajpcglclefindmkaj\/https:\/\/www.anses.fr\/fr\/system\/files\/BIORISK2020SA0095.pdf\">l\u2019A<\/a><a href=\"http:\/\/chrome-extension:\/\/efaidnbmnnnibpcajpcglclefindmkaj\/https:\/\/www.anses.fr\/fr\/system\/files\/BIORISK2020SA0095.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">N<\/a><a href=\"http:\/\/chrome-extension:\/\/efaidnbmnnnibpcajpcglclefindmkaj\/https:\/\/www.anses.fr\/fr\/system\/files\/BIORISK2020SA0095.pdf\">SES <\/a>qui vient de para\u00eetre et de donn\u00e9es de pr\u00e9valence sur la circulation des g\u00e8nes stx2a et stx2d dans les aliments \u00e0 risque, la d\u00e9finition actuelle des E. coli STEC hautement pathog\u00e8ne et les m\u00e9thodes analytiques
